Major Malfunction
I've had a major fault the last two times I've run SH4. Causes a video card error of some sort, and SH4 to stop working. I finally went into the Eventvwr and found this

Could AVG Free be missing it? Thanks!

What OS?
It's never a good idea to install games to the default folder (under Vista/Win7). Tried running without mods? Did it work before?

Sounds familiar, did you try posting in the SH4 Mods section?
Do any other applications have problems?

Since the issue only started recently with the one game (FSX might not be related) I'd say that it is a SH4 issue, maybe your game save is corrupt, pity you don't have a backup of your SH4 install, I assume you don't use Norton Ghost?
