You ignore the +. It just means that the 49 is added on. 49 is the code that assigns the number to Germany.

With regards to the extra (0), it just means that if you eliminate the +49 then you add a 0 on the front. Using UK numbers as I'm more familiar with them, +44 1234 567890 is the same as 01234 823792.

With regards to your number, according to Wikipedia, it should be +1 281-460-1111 for foreign calls (ie from non US/Canada). This is not location based so your number is same regardless of what country you're in as it's a mobile.
